Youth Evaluation Process
Evaluations are conducted by a highly trained Psychologist who is licensed by the State of Minnesota. The evaluation process often begins with a questionnaire that is filled out by the family. This packet asks for information regarding family information, where the child goes to school and description of the problems that prompted an evaluation. Any past child assessments, medical history or records of child development are usually requested. Family history and any past or present stressers should be included. An older adolescent or teen will have a separate questionnaire to be completed by them.
